ASIC Storm Investigation Questionnaire
Since May 2010, ASIC has provided Storm investors who have successfully applied for access to the Storm investor website with an ASIC Storm investigation questionnaire. The information investors provide in response to the questionnaire will assist ASIC with its investigations and commercial resolution discussions. The questionnaire includes questions which require investors to review their financial records to retrieve information from these records.
ASIC understands that answering the questionnaire may not be an easy process for some investors. ASIC officers will be travelling to the following regional areas to assist Storm investors with completing the questionnaire. ASIC officers will be available in these regional areas on the following dates:
TOWNSVILLE Monday 12 July to Friday, 16 July
CAIRNS Monday, 12 July to Friday, 16 July
MACKAY Monday, 19 July to Friday, 23 July
ROCKHAMPTON Tuesday, 20 July to Friday, 23 July
ASIC will also have staff available at its offices in Brisbane, Sydney and Melbourne to meet with Storm investors who require assistance to complete the questionnaire during business hours Monday, 12 July to Friday, 23 July.
Assistance will be provided to Storm investors by appointment bookings only. Bookings can be made by calling ASIC on 1300 300 630 (for the cost of a local call).
ASIC is also writing to Storm investors who have not elected to access information on the Storm investor website by hard copy to advise them of these arrangements.
Please note that in attending an appointment with ASIC to complete the questionnaire, investors will need to provide:
1. Proof of identification.
2. Their questionnaire, with as much completed as possible. ASIC officers will help investors with those areas that they are having difficulties completing.
3. Documents that are relevant to the questionnaire, for example, bank account statements (loan accounts and cash management accounts) and superannuation statements for the period of the Storm investment.
The ASIC officers will be available at these times solely to assist investors with completing the questionnaire. They cannot provide personal advice on particular circumstances of investors or discuss any aspects of ASIC’s Storm investigations or commercial resolution discussions.
